Annual Meeting Registration 2013

This form is for meeting registration purposes only. Hotel reservations must be made directly with the Loews Don CeSar Hotel.

For comprehensive meeting details, please review the NASD 2013 Annual Meeting section of the Web site in its entirety.

Online Registration: Online meeting registration will close at 5:00 p.m. EDT on Friday, August 16. Please note that the list of attendees provided in meeting packets will contain only the names of individuals who have registered by August 16.
On-Site Registration: After August 16, registration may only be accomplished on-site at the Loews Don CeSar Hotel, beginning at 8:30 a.m. on Wednesday, September 11. On-site registration will be assessed an additional fee of $25. To aid in on-site processing, you may choose to bring a completed registration form.

FEES

Annual Meeting: The basic Annual Meeting registration fee is $350.00. Each additional registrant from the same institution is eligible for a reduced rate of $325. The registration fee includes two luncheons, a dinner, and a continental breakfast.
Pre-Meeting Workshop for New Dance Administrators (if applicable): Registration and an additional fee of $55.00 are required for participation in this optional pre-meeting workshop. Please review the Pre-Meeting Workshop page for details.
On-Site Registration (if applicable): Attendees who have not registered by August 16 will be assessed an additional fee of $25.

Registering Individuals: A separate registration form and fee are required for each registrant. Each form should contain information about a single registrant only. When registering on behalf of someone other than yourself, please enter the name, title, address, and contact information of the registrant only in the "Registrant Information" section below.
Checking Out: During online checkout, transaction processing may take a few minutes after credit card information has been entered. Clicking the "Process Order" button more than once will result in multiple registrations and, therefore, multiple credit card charges. An automatic confirmation will be sent to the email address provided in your account information. If you do not receive an immediate confirmation, please ensure the accuracy of the email address in your account information, and/or check your spam folder.
If You Cannot Register Online: If you are unable to complete your registration online or do not wish to pay by credit card, you may download and print the hard copy 2013 Annual Meeting Registration form [PDF]. Complete and mail the form, along with payment, to the NASD National Office at the address indicated on the form in time to be received by August 16. Following processing, a confirmation of your registration will be mailed to you.
